Stress Echo Procedures:
If you would need to cancel this appointment we would need 12 hours notice of cancellation or a $200.00 (stress echo) will be billed. |
Echo Procedures:
If you would need to cancel this appointment we would need 6 hours notice of cancellation or a $150.00 will be billed. |
Dieticians:
If you need to cancel this appointment, we would need 6 hours notice of cancellation or a fee of $50.00 will be billed. |
Copays:
Co-pays are due at the time of your visit. If a co-pay is not received, you will be billed an additional $10.00 processing fee. We will send two notices to you with this fee applied, after the 2nd unpaid notice; this will be turned over to a collection agency. |
ARES Home Sleep Study Device:
The Ares home sleep study device is to be returned within 24 hours of the date of testing. If this device is not returned, the patient is financially responsible for $3,995.00. |
Holter Monitors:
Holter Monitors are to be returned 48 hours within the date of testing. If the device is not returned within 48 hours, a fee of $50.00 per day will be applied to patient’s bill. |
