Care coordination is a program designed to improve the delivery of care to patients with complex chronic medical conditions. Scarsdale Medical Group’s care coordination program provides our patients with assistance necessary to properly manage your medical conditions while improving your overall health status.
Our Care Coordinators will provide you with additional support and coordinate your care under the supervision of your primary physician. Communication with our patients upon discharge from the hospital is essential for a smooth transition back into the home.
We also provide transitional care to ensure safe passage from one health care setting to another or back into the community.
Our care coordination team will work closely with you to schedule timely follow-up appointments with your provider, discuss any ongoing health concerns, identify possible medication discrepancies, provide education, ongoing support and disease management to improve your health as well as identify community resources or other support such as long term care, home health services or transportation.

Julie joined Scarsdale Medical Group to help further develop our Care Coordination Program.
Julie is a Registered Nurse having received her Bachelor’s of Science in Nursing from New York University. In addition, she holds a Master’s in Public Health from Hunter College of the City University of New York where she specialized in Community Health Education and is an Accredited Case Manager. Before joining SMG Julie worked in the hospital setting as well as in home care providing clinical care coordination for both children and adults. Julie is based at our 550 Mamaroneck Avenue location and can be reached on Wednesdays and Fridays at (914) 723-8100 extension 349.
Dona joined Scarsdale Medical Group in August 2014. Dona has joined our Care Coordination team as to better improve the quality of care transition for our SMG patients. Prior to this she has worked in both the Hospital setting as well as Skilled Nursing Facilities in different medical positions for the past twenty years. Her goals are to assist wherever necessary to improve the well-being of our patients. Dona graduated from Westchester Community College in 2006. Dona is at our 550 Mamaroneck Avenue location and can be reached Monday to Friday at (914) 723-8100 extension 308.
