Scarsdale Medical Group is committed to providing the highest quality health care while maintaining the privacy and security of your health information. Our Notice of Privacy Practices describes the ways in which we may use or disclose your protected health information.

The Scarsdale Medical Group, LLP has an ongoing commitment to protect the privacy of our patients’ personal data and health information, as required under HIPAA (Health Insurance Portability Accountability Act) enacted by the United States Congress in 1996. To assist in this effort it is important that you inform us of any changes in your contact information and contact preferences. Please take advantage of our patient portal to update all necessary information. If you choose to wait until you come into one of our offices for a visit, please allow 10-15 minutes before your appointment time to provide any updates. If you have not updated your contact information within the past year, you may be asked to do so at the time of your next visit.

When you come for an office visit, we ask that you bring the following items with you:

  • Your current health insurance card;
  • Your driver’s license or other government-issued picture ID; and
  • A utility bill or other similar correspondence showing your current address, if your driver’s license or other photo ID does not have that information.

If you are a minor (less than 21 years old), your parent or guardian should bring the information listed above.

Another important step we are taking to protect your privacy is to verify during each telephone contact your name, date of birth, telephone number and home address. In addition, we will provide information concerning your treatment only to you or to specifically designated family members, friends, or caregivers. Therefore, if you would like a family member, friend, or caregiver to be able to obtain information regarding your treatment, you must list each individual on the Scarsdale Medical Group, LLP’s Designation of Disclosure form. This form can be found in our patient portal or obtained from a receptionist.
